For our admissions department, call 844-226-8855. Be sure to have the following information ready before you call:
- Comprehensive Clinical Assessment
- Download and complete the Referral Form.
- Copies of the front and back of the client’s Medicaid card
- Email the documents above as attachments to admissions@childrenshopealliance.org
