What is Healthy Transitions?
Healthy Transitions is a program designed to support youth and young adults in Iredell County, ages 16 to 25, with mental health challenges as they navigate key life transitions, such as finishing school, starting a job, and/or living independently.
Program Details
Healthy Transitions is a grant-funded program offering free services through a combination of in-person and virtual sessions. The duration of participation is tailored to individual needs and goals, and together, we will create a plan that works best for you. Additionally, Healthy Transitions can complement existing services and collaborate with your current therapist or counselors to ensure a coordinated approach.

Youth Advisory Council

Make a Difference!
The Youth Advisory Council (YAC) is a group of young people (ages 16-25) who are passionate about making a difference in Iredell County. YAC members use their voices, ideas, and lived experiences to help shape programs and create positive change for other youth and young adults in the community. If you care about mental health, advocacy, and building a stronger support system for young people, this is the place for you!

YAC Details
- You will be compensated for your time and experience ($30 gift card per meeting you attend)
- Meetings will be held every 2-4 weeks and will be a mix of virtual and in-person
- Who can join? Anyone ages 16-25, and living in Iredell County or very familiar with the needs and challenges of young people in Iredell County

What You’ll Do As a YAC Member
- Help plan fun and meaningful community events for young folks
- Work on youth-led service projects
- Give feedback on materials and programs created by the Healthy Transitions program
- Advocate for what young people really need
- Connect with others who care about making a difference
